HEALTH ABILITY has been requested by icare, or one of icare claims agents (i.e. EML, GIO, Allianz, QBE or Gallagher Basset) or a specialist insurers (e.g. Hii, Trinity, State Cover Mutual, Coal Mines Insurance, etc), to support you as part of your workers compensation claim & your recovery from injury or illness.

For your HEALTH ABILITY Rehabilitation Consultant to support you, your insurance company and SIRA require you sign this Consent form.

I agree to participate in return to work process and workplace rehabilitation services provided by HEALTH ABILITY. I authorise HEALTH ABILITY to obtain and release rehabilitation related information from relevant stakeholders to support my workplace rehabilitation. I understand this information will be utilised to help manage my workers compensation claim and support my return to work. I understand this information will be respected privately and confidentially in line with the Australian Privacy Principles. I acknowledge I can request a copy of the Privacy Policy and understand I can change or cancel my consent.

 RC's Role in Helping You

Rehab Consultants (RC) are allied health professionals who are trained and accredited to provide you expert assistance and support to help you recover from your injury, illness, or condition.
Rehab Consultants have tertiary qualifications, experience, and expertise to provide advice, assessment and intervention when needed to help you safely and comfortably return to work.
They also provide you with support by communicating with other professionals (for example your doctor, physio, employer/supervisor, case manager, icare, etc.) involved in your return to work process.
If HEALTH ABILITY is unable to assist, an injured worker can select an alternate rehabilitation provider.
